Clientele qualification: Children in Conflict with the Law who are above fifteen (15) but below eighteen (18) years of age and Children above 12-15 years of age who commit a serious crime or Oftener (repeat offenders)
Provision of Emergency Assistance (Medical)
Requirements:
- Doctor’s Prescription with License number
- Barangay clearance of the patient or parents if patient is minor
- Voter’s ID or Botika sa Masa Card Fee Php 20.00 – Renewable once a year
- Botika sa Masa Card
Procedures:
Note: If Botika sa Masa is available, no need to comply the first requirements
Step 1. Client applicant/family members proceed to City Social Welfare and Development Office to seek assistance
Step 2. Interview clients/family member/Comply and submit requirements
Step 3. Issue Certificate of Eligibility and have it signed by City Social Welfare and Development Officer or representative
Step 4. Issuance of Botika sa Masa Card
Step 5. Instruct client to proceed to City Health Office then to City Mayor’s Office for signature
Step 6. Claim medicines at the City Health Pharmacy
NOTE: Cut off Time for Pharmacy is at 11:00 am and 4:00 pm
Provision of Emergency Assistance (Burial)
Requirements:
- 3 Copies of Death Certificate
- Barangay Certificate of the Deceased
Procedures:
Step 1. Proceed to City Social Welfare and Development Office to seek assistance
Step 2. Interview clients/family member/comply and submit requirements
Step 3. Issue Certificate of Eligibility and have it signed by City Social Welfare and Development Officer or representative
Step 4. Proceed to City Mayor’s Office for signature
Step 5. Client proceed to Funeral Service Office for the release of Coffin and other burial services available
Provision of Emergency Assistance (Hospitalization)
Requirements:
- Doctors Impression
- Barangay Clearance for patient or parents if patient is minor
- Voter’s ID or Botika sa Masa Card fee Php 20.00 (renewable once a year)
Procedures:
Step 1. Proceed to City Social Welfare and Development Office to seek assistance
Step 2. Interview clients/family member/comply and submit requirements
Step 3. Issue client 3 copies of Certificate of Eligibility and have it signed by City Social Welfare and Development Officer or representative
Step 4. Issuance of Botika sa Masa Card
Step 5. Proceed to City Health Office then to Mayor’s Office for signature
Step 6. Go to City Health Office to submit 1 copy of the Certificate of Eligibility then to City Social Welfare and Development Office to submit the 2nd copy of the Certificate of Eligibility
Step 7. Submit the last copy of the Certificate of Eligibility to the Social Services Unit in the hospital
Provision of Emergency Assistance (Financial Assistance)
Requirements:
- Medical Certificate original copy or certified true copy
- Hospital Bill original copy or certified true copy
- Barangay clearance of client
- Photocopies of client ID
- Case Findings
Procedures:
Step 1. Proceed to City Social Welfare and Development Office to seek assistance
Step 2. Interview clients/family member/comply and submit requirements
Step 3. Prepare Case Findings for encoding and attachment of necessary document
Step 4. Wait for concerned persons in the office (area Supervisor and Department head) to sign papers
Step 5. Receive of Financial Assistance
Provision of Emergency Assistance (Referral Assistance)
Requirements:
- Medical Abstract original copy
- Hospital Bill original copy
- Barangay clearance
- Photocopy of client’s ID
- Case Findings/Study
- Voter’s ID
Procedures:
Step 1. Proceed to City Social Welfare and Development Office to apply for scholarship
Step 2. Interview applicant and parent or guardian
Step 3. Comply and submit requirements
Step 4. Wait for date receiving the documents
Step 5. Carry the papers to the Agency Concern
Adoption Services
General Requirements for the Adaptor:
- 18 years old to 48 years old
- Has legitimate income
- Good moral character
For Independent Adoption (children who were entrusted to couple without legal papers beyond the 3rd degree of consanguinity)
Requirements:
- Birth certificate
- Deed of Voluntary Commitment notarized by a lawyer (through counseling of birth mother before the lawyer)
- Medical certificate of both child and Prospective Adoptive Parents
- Immunization record of child
- NBI and barangay clearance of PAPS
- Employment certificate or proof of income of PAPS
- 3 characters references for PAPS
- Attendance of Adoption Forum of PAPS
- Pictures of Child with PAPS and family (old and recent)
- Marriage license
- Affidavit of Guardianship
- Psychological evaluation of child aging 5 years old and above
- Making of Child Study Report
- Issuance of CDCLAA (Certificate of Child Legally Available for Adoption
- Home Study Report
- Matching Conference
- Issuance of ACA and PAPA (Affidavit of Consent to Adoption, Pre-Adoptive Placement Authority
For Abandoned and Neglected children (children who left in the hospitals, somewhere)
Requirements:
- Police blotter
- Tri media publication (newspaper, radio and postal when address of biological parent is indicated)
- Certification from Tri Media Publication
- Medical certificate of both child and PAPS
- NBI and barangay clearance of PAPS
- Dental Aging result of Child
- Old and recent picture of the child
- 3 character references for PAPS
- Immunization record of the child and other health documents
- Medical Certificate of both Child and PAPS
- Employment Certificate or Proof of income of PAPS
- Marriage license
- Affidavit of Emergency Guardianship
- Attendance to Adoption Forum of PAPS
