PRC – Mobile Renewal of Professional Licenses 2016
Professional Regulation Commission (PRC) will hold a Mobile Renewal of Professional Licenses
8:00am to 4:00pm Saturday December 03, 2016
8:00am to 11:00am Sunday December 04, 2016
Venue is the Sofia Soller Sinco Hall on the Main Campus of Foundation University in Dr. Miciano Road, Dumaguete City.
Requirements for Renewal of Professional Identification Card
- 2 pieces passport size ID picture, clear, close up, white back ground, without eyeglasses in complete name tag (scanned pictures are not accepted).
- 1st picture to be pasted in the renewal form
- 2nd picture to be given to PRC personnel during transaction
- Photocopy of expired PRC ID Card
Requirements for Renewal of Professional Identification Card at the same time change of name due to status:
- 3 pieces passport size ID picture, clear, close up, white back ground, without eyeglasses in complete name tag (scanned pictures are not accepted).
- 1st picture to be pasted in the renewal form
- 2nd picture to be pasted in the change status form
- 3rd picture to be given to PRC personnel during transaction
- Original NSO Marriage Contract, if unreadable attach photocopy of LCR. (Local Civil Registrar)
